New Student Health Information Portal

Dear Parents,

Welcome to Hebron Academy.  Our health services utilize Magnus Health SMR (Student Medical Record).  It is a web-based system that allows you to continuously access your child’s medical records as well as the ability to make updates as needed.  On June 2, you will have access to your Magnus Health SSR account through the Hebron Academy website.  We ask that you enter the health information required (called requirements) for each child attending Hebron Academy no later than August 1.  Some requirements have an early due date of July 1.  Be sure to look through and make note of all due dates.

To access your Magnus Health SMR account, please visit the school website at www.hebronacademy.org , the follow the directions below

• Click on myHebron in the top right corner and login in using your parent username and password received in this mailing. Be sure to remember your username and password as you will need for the duration of the time your student is at Hebron Academy.

• Click Resources at the top of the page.

• Click on either Parent Resources for Upper School if your student is 9th-PG year or Parent Resources for Middle School if your child is in 6th-8th grade.

• Click on Medical Records.

• Click Magnus Health Link and follow the instructions to complete the requirements in the Student Medical Record.  It is helpful the first time in to look over each requirement to understand what you will need to do and when they are due.

• Please note the annual physical exam form and the immunization form will need to be printed out.  On the annual physical form there is a section the parent fills out to take to the student's provider.   The provider must complete the physical and the immunization form and sign both.  When complete please upload to the requirement.

Timely completion and submission of all requirements is vital so that the health service team is prepared for your student's arrival.  You will receive weekly reminders as long as any requirement is outstanding in your account.  A student will not be permitted on campus without all requirements complete.

Please we ask that only parents access Magnus and complete any requirement.

Please have your student’s primary healthcare provider complete the form and date it for the day the exam was done.  A physical is good for 13 months so be sure to schedule your student to have theirs done yearly, even if it falls during the school year. If it becomes due during the school year, we will send you a reminder.  Please be sure to always use the form provided.  We will not accept any other form.

If your student is on a prescription medication, the prescription medication form must be filled out and signed by the provider who prescribed it.  One form per medication please.  Be sure to read the information in the Community Handbook on medications, supplements and naturopathic medications,when it comes out this summer.
