Returning Student Health and Medical Information

Dear Parents,

Hebron Academy Health Services will continue to utilize Magnus Health Student Medical Records for the upcoming school year to collect and store your student's health and medical information.  You will be able to access your account beginning June 2.  Some requirements will be due by July 1, all requirements are due no later than August 1.  You will receive weekly email reminders until all requirements are done.

To access your student's SMR, please visit the school website at www. Hebronacademy.org and follow the steps below.

  • Click on myHebron in the top right corner and login in with your parent username and password (if you have forgotten your login information, please email Heidi Collins at hcollins@hebronacademy.org

  • Click on Resources at the top of the page

  • Click on Parent Resources for Upper School if your student is in grade 9th- PG or Parent Resources for Middle School if your student is in 6th - 8th grade.

  • Click on Student Health Services

  • Click on Medical Records

  • Click on Magnus Health Link and follow the instructions.  We recommend that the first time you log in this year, you go through and review all requirements and what is needed for each as well as the due date.

  • Please note the annual physical exam form and the immunization form will need to be printed out.  On the annual physical form, there is a section the parent fills out to take to the student's provider.   The provider must complete the physical and the immunization form and sign both.  When complete please upload to the requirement.

Timely completion and submission of all requirements is vital so that the health service team is prepared for your student's arrival.  You will receive weekly reminders as long as any requirement is outstanding in your account.  A student will not be permitted on campus without all requirements complete.

We ask that only parents or guardians access Magnus and complete any requirements.

Please have your student’s primary healthcare provider complete the form and date it for the day the exam was done.  A physical is good for 13 months so be sure to schedule your student to have theirs done yearly, even if it falls during the school year. If it becomes due during the school year, we will send you a reminder.  Please be sure to always use the form provided.  We will not accept any other form.

If your student is taking a prescription medication, the prescription medication form must be filled out and signed by the provider who prescribed it.  One form per medication, please.  Be sure to read the information in the Community Handbook on medications, supplements, and naturopathic medications.
